The reviewer needs real authority
A human reviewer should be able to understand the relevant decision context, challenge the AI output and change or stop the outcome. A nominal sign-off that cannot alter the result is weak oversight.
Oversight must match impact
Uses affecting rights, employment, services, money, safety or vulnerable people may need stronger independent review, accessible alternatives and contestability than low-risk productivity uses.
Record the operating control
The governance record should show who performs the review, when it occurs, what information they see, what override authority exists and how complaints or appeals reach a qualified person.
